history or chief complaint underwent MC screening (score range 0-5, failure < 4) by rooming nurses. A prospective chart review of the electronic medical record (EMR) was conducted over an 18 month post-screening period. Percentages were calculated for MC failure and for each diagnostic/treatment intervention performed in those failing the MC. Results:A total of 649 patients (male1⁄4277; female1⁄4372) underwent MC screening, and the average age was 78.3. The overall MC failure rate was 37.3%. Of the 242 subjects who failed the MC, 31.5% of patients received follow-up cognitive screening within a mean period of 76 days and had the following failure rates on the MOCA (81.3%), MMSE (42.9%), and Kokmen (40.0%). Only 29.3% of subjects endorsed symptoms of memory impairment. Additional interventions included referrals for neuropsychological testing (7.9%), CT or MRI neuroimaging (4.1%), and dementia specialty clinic (6.2%). A provisional diagnosis of dementia was rendered in 13.2% of cases and 11.6% were prescribed cholinesterase inhibitors or memantine. Conclusions: Undiagnosed cognitive impairment is more common in general neurology compared to primary care populations, yet MC performance had only a modest impact on neurologist decisionmaking with regard to dementia evaluation and treatment. Further investigations are needed to examine factors underlying the limited work-up of neurology patients performing abnormally on the MC.
