low income neighborhoods in a Midwestern city in the U. S. We used a deliberative process to determine how the existing PCMH PwD could best be adapted to meet the needs of a well described neighborhood of 5,604 residents, 94% of whom are African American with 39% below the poverty level, 19% aged 65 or older, and an estimated 200 people with dementia. The deliberative process initially involved staff members of a nonprofit senior center and community organizers to gain their input on the impact of dementia in the community, current dementia services, and unmet needs. Informed by this initial work, the research team and community organizers used a deliberative process with a sample of neighborhood residents to determine general knowledge about dementia and its causes and consequences, the needs of people with dementia and their caregivers, attitudes about how the needs should be met, and characteristics of the healthcare and social services professionals who should assist in meeting the needs and the approaches they should use in meeting the needs. Findings suggest that successful implementation of the PCMH PwD in diverse communities will require substantial changes in policies and procedures to accommodate cultural and family values.
