Background: The IDEA study screening tool for dementia was developed as a brief screen for use in low-resource settings and populations with low levels of formal education. The tool was initially field tested in a rural community-based sample in Tanzania, and has been subsequently validated in a hospital sample in Nigeria. Methods: The screening instrument was administered to 100 consecutive inpatients aged 65 and over admitted to Mawenzi Regional Hospital, in Kilimanjaro region of Tanzania. In addition, 108 consecutive outpatients attending the geriatric medicine clinic at the same hospital were also screened. All inpatients were subsequently fully assessed by a research doctor who was blind to the screening tool results. All screen positive outpatients and a random sample of screen-negative patients were also assessed by the same method. The assessment included a psychiatric and cognitive examination alongside informant history and neurological examination. Dementia and delirium were diagnosed according to both DSM-IV and ICD-10 criteria. Results: Provisional results indicate that the area under the ROC curve was 0.839 for dementia and 0.897 for dementia and delirium combined. Using a cut-off of 7 sensitivity was 0.9 with specificity of 0.73 for dementia and 0.84 and 0.87 for dementia and delirium combined. Conclusions: This six-item brief dementia screening instrument performed well in this population and should prove useful in screening for dementia in this setting. Further testing in other low-resource and community settings is required.
