geriatrician prompt a counseling referral. A counselor then conducts a telephone screening interview with the PCG and sets up an initial counseling assessment with the PCG, PCG + PWD, or the PCG + PWD + FM. Short term counseling of up to 12 sessions as well as one of two closed groups of 6-8 sessions each (a caregiver support group and an “early journey” support group for the PCG + PWD) are then offered depending on the PWD, PCG, and FM’s needs. The involved geriatricians and counselors meet bimonthly for a multidisciplinary conference. Results: In the first three months of operation, the WFBH MAC counseling program received 30 referrals. The numbers of PWD, PCG, and FM receiving individual, couple, family, and group counseling will be described. Conclusions: Awellnessbased counseling program for MCI/dementia patients and their caregivers/ family members is complementary to a comprehensive memory assessment clinic and helps meet the medical and mental health needs of this growing population.
