Memory and communication training in dementia to improve the transition from home to residential care

is increasingly community based and delivered by a multi-disciplined workforce. This project aimed to produce Dementia Care Pathways, informed by evidence based guidelines, to resource the health and ancillary workforce in the support of people with dementia and their carers. Methods: Existing national and international published guidelines were reviewed using the Appraisal of Guidelines for Research and Evaluation Instrument (AGREE) instrument and appropriate evidence based guidelines and inform development of dementia care pathways. Focus Groups with Carers, General Practitioners, Nurse Practitioners, Adult Mental Health Workers and Service Providers for Ethno-specific Groups also informed the development. Results: To guide Support Workers, Allied Health Professionals and General Practitioners a systemic Dementia Care Pathway supported by evidence based Guidelines was designed. These Pathways incorporate three phases of care: 1. Recognition, Assessment and Diagnosis, 2. Post Recognition Monitoring and Care, and 3. Late Stage Pathways. Conclusions: The pathways, guidelines and a complementary education package offer an effective tool in promoting dementia care standards and responding to increased presentations of dementia. The pathways, together with the guidelines and a complementary education package have been incorporated in the future planning of Queensland’s Home and Community Care (HACC) program. The project’s industry partners BlueCare, RSL Care and The Prince Charles Hospital Health District are committed to their use, and staff from both the community and acute sectors have undertaken pathway education. of General Practice in Queensland have been invited to establish a link to this resource.
