Acute appendicitis is a known surgical emergency. It is rare for port site hernia to contain incarcerated inflamed appendix.Case presentationWe describe a distinctive case of incarcerated appendicitis in port-site hernia following laparoscopic cholecystectomy. Acute appendicitis was pre-operatively CT diagnosed and appendectomy was performed followed by primary repair of the hernia defect. Awareness of atypical presentation and possible complications may raise the clinical suspicion index to ask for early CT
