Patients with trigeminal neuralgia undergo various investigations in-order to rule-out suspected underlying causes. MRI has proven to be the imaging modality of choice when trigeminal nerve pathology is suspected; it also provides useful guidance in-terms of management decisions.We discuss two middle aged ladies who presented with trigeminal neuralgia, MRI in both of them revealed the presence of intracranial extra-axial mass lesions with the characteristic features of meningioma causing trigeminal nerve compression and explaining the cause of neuralgia
