235 Stress ulcer in childhood

From 718 children (4 days-12 years of age) died in ICU in a 7 years period, 55 cases (7.5%) presented acute erosions of tbc gastric and/or duodenal mucosa, having as clinical expression the cmesis in 20/55 cases (77% from all the em&s in the studied period) and 1 cast of perforation. Infants represented 87.2% of the group, with predominance of the first 3 months (18/55). The lesions were found especially in the acid-secreting portion of the mucosa, most of them limited to the mucosa, with an aspect of single lesion in 5 cases (3 gastric and 2 duodenal localisations) and multiple lesions in 50 cases. The involvement of muscularis mucosae was found in 6 eases (2 duodenal k&is&ions). Stress ulcer represent+ the complication of bronchopncumonia (17159, scverc diarrbcic disease (15/55), septicemia (IYSS), Reye syndrome (3/55), mcuinco-cercbral haemorrhagc (X5.5), acute renal failure (3/55) and hydrocephalia (2155). Continuous neutralisation of the gastic activity with a saline solution (gastric pH > 6) and an intravenous infusion of cimctidine increase the possibility to stop the haemorrhage and to recover the stress ulcer.
