Ten postpubertal women with congenital adrenal hyperplasia (CAH) had significant vaginal stenosis after at least one previous attempt at vaginal reconstruction. Two women had had two unsuccessful surgical attempts to obtain a vagina of adequate size to allow sexual relations. This report describes a surgical procedure to exteriorize the vagina. With post‐operative vaginal dilatation and proper patient counseling, this combined approach has resulted in a vagina of adequate size to allow comfortable intercourse.
