Tarlov or perineurial cysts are nerve root lesions common in the sacral region. These cysts were reported to mimic adnexal masses on TVS. Materials and methods: 4 women referred for adnexal mass. TVS: normal uterus and ovaries + multiple bilateral cysts containing mildly echogenic content located posteriorly to peritoneum suspected Dx of Tarlov cyst. Results: Tarlov cysts was Dx subsequently confirmed by either CT or MRI. EP 30.01: Trans-vaginal sonography diagnosis of Perineurial (Tarlov) cysts
