Clozapine is primarily used for patients with treatment‐resistant schizophrenia. Despite the drug's efficacy, its usefulness is limited by the occurrence of significant side effects, such as neutropenia and agranulocytosis, which occur in 1 to 2% of patients. The presence of an immunocompromised state in a clozapine‐treated patient could put the individual at a higher risk of infection. The following case described the occurrence of clozapine‐associated cytomegalovirus (CMV) colitis in a patient who was immunocompetent.
