Abstract This paper describes the failure of the pressure shell of an interconnecting pipe between the secondary reformer and primary waste heat boiler that led to a fire in Ammonia Unit I after 13‐and‐a‐half years of operation. The fire incident resulted at KRIBHCO in a plant shutdown of 15 days. It also describes analysis, the repair work carried out to put the plant back in service in the least possible time, and the remedial measures taken.
