The historical development of blood immunoreactive trypsin as a screening procedure performed on neonatal dried blood is described. Our experience of false positive, and false positive results using the original procedure has led to some useful modifications which reduce these errors to an acceptable level. A satisfactory national screening programme has been in place in New Zealand for 10 years. Measurement of the LF 508 gene may further reduce the errors encountered in screening.
