Abstract We describe a 12‐year‐old boy with Pneumocystis carinii pneumonia complicated by extensive pneumatocele formation as the presenting manifestation of human immunodeficiency virus infection. This report emphasizes the importance of considering Pneumocystis carinii infection in the differential diagnosis of pneumonia associated with pneumatocele formation in children.
