The Year of Care initiative aims to transform the annual review into a collaborative care planning consultation based on a partnership approach. It has been piloted across three centres in England. This paper describes the key drivers that created the impetus for the development of the approach. The care planning model developed by Year of Care, ‘The House Model’, is presented and the process of the care planning consultation described. The theoretical underpinnings and supporting evidence are presented for each of these as well as the philosophical assumptions and values that underpin the programme.
