Venous thrombosis of a free flap is a serious complication in microsurgery. Several agents with the ability to dissolve an occluding thrombus exist. Recombinant tissue plasminogen activator (rt‐PA) seems the most effective. We present our experience with a procedure that was successful in elimination of the occluding thrombus in two patients. © 2004 Wiley‐Liss, Inc.
