The authors present a case of free microsurgical transfer of combined flexor carpi radialis muscle and radial fasciocutaneous forearm flap in reconstruction of facial paralysis associated with severe ipsilateral skin scar deformity after removal of a giant cavernous hemangioma 22 years previously. The flexor carpi radialis muscle was used to reanimate the paralyzed face, and its fasciocutaneous radial forearm part restored overlying hemifacial skin, with good functional and rather pleasing aesthetic results.
