We describe a patient with clinical signs of parkinsonism showing a paradoxical response to apomorphine injection. We discuss possible pathogenetic mechanisms with regard to the literature and suggest the diagnosis of a striatonigral degeneration at an early stage. © 2002 Movement Disorder Society
