Despite significant strides in hepatitis B virus (HBV) vaccination and prevention in the United States and throughout the world, liver transplantation remains the only curative option for patients with complications of chronic HBV, including severely decompensated cirrhosis, hepatocellular carcinoma (HCC), and severe acute presentations including acute liver failure. The recently published 2018 American Association for the Study of Liver Diseases (AASLD) Hepatitis B Guidance provides an evidence-based approach to screening, prevention, diagnosis, and clinical management of patients with HBV. In contrast to the 2016 HBV guideline and meta-analysis, this guidance document also incorporates expert consensus along with formal review of the published literature and the World Health Organization guidance on HBV. Thus, this guidance document provides a crucial synthesis of information to guide practitioners in the care of patients with HBV. However, given the limited data available for HBV management specifically for liver transplant candidates and recipients, there is a lack of high-quality evidence in many aspects of the care of these most complex patients.
