There is no consensus regarding duration of intubation prior to elective tracheotomy. This report emphasizes the importance of serial examination of any patient with long term intubation (>7 days) who develops persistent voice alteration with findings of posterior glottic granulation tissue. Early definitive surgical intervention for interarytenoid adhesions and posterior glottic webs is necessary for acute change in respiratory or voice characteristics. Botox use may augment surgical intervention by reduction of contact time of bilateral granulation tissue. Interarytenoid adhesions secondary to intubation trauma are thought to cause vocal cord abductor immobility secondary to contraction band formation.
