A 73‐year‐old man presented with progressive painless scrotal swelling without systemic, urinary, or anorectal complaints. The scrotal lymphedema was due to massive bilateral metastatic replacement of the superficial inguinal lymph nodes. The primary tumor was an inoperable, rapidly fatal adenocarcinoma arising from the anal glaänads of the upper canal. Thus, in the evaluation of scrotal swelling of obscure etiology in older patients, occult carcinomas of the anal glands must be considered and appropriate steps taken to establish the diagnosis.
