A case of hemangioendothelioma of the penis is presented. It was treated initially by excision and postoperative irradiation to the primary site. Systemic chemotherapy and radiotherapy to the thoracic spine was administered in the belief that the patient had osseous metastases in the spine. The patient was subsequently free of disease for more than five years at which time metastases appeared almost simultaneously in the extradural spinal column, right 12th rib, and lungs. A possible explanation for the peculiar distribution of these metastases is presented. A review of the literature reveals no more than 30 other cases, none of which showed a similar clinical course, namely later appearing metastases after a five‐year disease‐free interval.
