A successful right hepatic lobectomy for a large solitary liver metastasis of a leiomyosarcoma was performed 8 years after removal of the primary lesion in the right colon. An anomalous arterial supply to the medial segment of the left hepatic lobe was encountered during the dissection. Clues to the presence of the anomalous blood supply and the rationale and results of hepatectomy for metastatic neoplasms are discussed briefly. An aggressive approach to surgical therapy is advocated in selected cases.
