One of the characteristics of good medical care is accurate recording of findings. Reasons for the careful and full documentation of oral cancer are reviewed. The components of adequacy are verbal description, mensuration, diagram, photography, and, when indicated, radiography. Details of each of these steps are presented, with emphasis on the value of simple line diagrams, upon which extent of tumor is indicated by a standardized method. It is recommended that national organizations concerned with head and neck oncology consider adoption of a uniform method of diagramming the extent of oral tumors.
