Providing health care outside the physical environment of the treating hospital has been practised for many years. Pharmacy involvement in such systems has been variable, ranging from supply of basic medications to providing individualised dosage forms and clinical support. Patients may reside at home, often with care being shared with the general practitioner and community nursing and pharmacy services, or in a remote hospital or other institution. Models of pharmacy services to support such arrangements vary greatly, often reflecting local funding and structural arrangements for providing health care. Issues to be addressed when providing pharmacy services in such circumstances are identified and Australian resources for information and support are noted.
