Mohamed-Kalib et al. illustrate 2 important caveatsto penicillin skin testing (PST): (1) there is an excep-tionally rare potential for resensitization, a phenom-enon in which a previously reactive patient is proventolerant, then develops sensitivity and has a positivePST; (2) consider repeating PST prior to a parenteralb-lactam prescription in patients who previouslyreported severe anaphylactic reactions.Our negative predictive value of 100% does notabate the tentative concern for resensitization.
