Significant sonic shadowing originating from a structure within the hepatic parenchyma is unusual and often denotes pathology. The differential diagnosis (choledocholithiasis, air in the biliary tree, calcified tissue, foreign bodies, and shadowing from normal structures) is reviewed and examples are presented.
