Summary The anaesthetic management of patients with large intrathoracic tumours can be extremely challenging. Critical respiratory and haemodynamic compromise can occur as a result of tumour compression of the airway or great vessels. The potential for massive haemorrhage further complicates intra‐operative management. The multidisciplinary management of a giant mediastinal teratoma occupying the whole left thoracic cavity in a paediatric patient is presented.
