Medication reconciliation is in the forefront of medication safety programs, both in Australia due in part to updated accreditation standards, and globally. The first international meeting for hospitals participating in the WHO's High 5s Project was convened in Geneva in October 2012. The meeting was attended by delegates from hospitals in 6 countries participating in 2 of the WHO patient safety initiatives: medication reconciliation and correct site surgery. Networking with European colleagues provided an opportunity to exchange ideas and collaborate to address common medication safety issues.
