To assist gynecologists in their evaluation of sterilization procedures, sterilization of the not recently pregnant patient through a posterior colpotomy is discussed. Patient selection, preoperative and postoperative procedures, surgical technique and difficulties, effectiveness and complications are detailed. Surgical difficulties and early postoperative complications are evaluated from data reported to the International Fertility Research Program on 398 patients sterilized by classical methods through a posterior colpotomy. It appears that skilled vaginal surgeons can perform sterilization through a colpotomy with clinically acceptable rates of surgical difficulties and complications. Some investigational techniques of tubal occlusion performed through a colpotomy may result in lower rates of morbidity.
