Amrinone is a bipyridine derivative that exerts systemic pulmonary vasodilatory effects while also being a positive inotrope. Amrinone is used frequently to treat pulmonary hypertension, congestive heart failure, and postoperative heart failure. Because there is a direct relationship between plasma amrinone levels and cardiac index, 1 knowledge of the pharmacokinetics of amrinone would be beneficial in patient management. Despite its widespread use in critically ill adults and increasing use in critically ill pediatric patients, little information is available about the dosing and pharmacokinetics of amrinone in pediatric patients. 2 Our objective was to determine the pharmacokinetics of amrinone in the critically ill pediatric patient as a guide to drug dosage requirements.
