This article comments on another Journal article written by James C. Vaughn, “Water Plant Safety,” July 1962, p. 789 about three significant principles that are necessary for the successful administration of an accident prevention program at a water treatment plant. This article provides two examples of serious accidents resulting from responsibility failures by management and by employees that illustrate the importance of maintaining safety responsibility and participation by both management and employee. This article also comments on the third point of significance in Vaughn's paper, calculated risk.
