During 1983, the city of Grand Rapids, Michigan experienced coliform bacteria contamination in the distribution system for four days in May and for 13 days in June. This article describes the supply and facilities, immediate control measures, the investigation procedure, and follow‐up activities. Four system deficiencies were identified as contributors to the contamination. At the time of publication, the city was continuing daily monitoring of the system and maintaining a chlorine residual of 2 mg/L.
