Erratum: In this report, the text clearly states that the age of onset in the reported family was in the fourth or fifth decade of life. In Table 1 the age of onset is reported as the “2 nd ‐ 3 rd decade.” The age of onset stated in Table 1 is incorrect and should be “4 th ‐ 5 th decade” as stated in the text.
