This article presents three tendencies (hubris, hamartia, and anagnosis) to explain why organizational personnel are prone to fail. All people are subject to these tendencies. Few organizations or individuals understand them, however, and fewer still know how to manage them. The article employs short case studies to define them and offers solutions on how to overcome their impact.
