Necrosis of an allograft's renal pelvis and ureter is an uncommon but potentially devastating complication of renal transplantation. Although its occurrence may result in significant morbidity—including graft loss—or mortality, reconstruction of the collecting system may save the transplanted kidney. We present a case of successful renal allograft salvage despite necrosis of the renal pelvis and ureter, utilizing a modified ureterocalicostomy.
