Antihypertensive efficacy of metolazone

The antihypertensive etfects of metolazone and chlorthalidone were compared in 57 nonedematous hypertensive patients using a double‐blind technique. Metolazone was followed by significant reductions in arterial blood pressure at all doses (1.0, 2.5, and 5.0 mg daily), which were comparable to those of chlorthalidone, 100 mg daily. Although the chlorthalidone‐treated patients lost an average of 8.7 pounds after 24 weeks of therapy, there was no change in body weight after metolazone. Side etfects were similar after both; hypokalemia was equally frequent. It was concluded that metolazone in doses of 1.0, 2.5, and 5.0 mg daily was as etfective as chlorthalidone, 100 mg daily, and that 1.0 mg was probably a ceiling dose in patients without frank edema. M etolazone appeared to otfer no advantage over chlorthalidone.
