Highlights in the history of the development of successful and specific treatment for pernicious and other megaloblastic anemias are reviewed. The evolution of knowledge concerning the clinical and physiological basis for their separation into those due to vitamin B 12 and to folic acid deficiencies, respectively, is discussed, and the consequent biochemical abnormalities related to megaloblastic erythropoiesis are briefly presented. The importance of informed collaboration between the medical profession and the pharmaceutical industry for the future initiation, development, and appraisal of new drugs is emphasized.
