Few drugs are more widely used than local anesthetics, and few have more predictable effects. Their wide usage stems from the apparent ease of administration, their therapeutic effect in relieving pain, and their consequent ready acceptance by the patient. Yet it is probable that no other class of drug is used so ineptly and results in so many immediate adverse reactions. Both failures and complications are brought about by lack of understanding of a few simple pharmacologic facts and technical principles. It is the aim of this article to review these so that local anesthesia may be more safely and successfully utilized.
