Diagnostic error is a critical focus of the modern patient safety movement and pathologists are at its core. In this issue, Poller and colleagues tackle the challenging business of diagnostic error in thyroid fine‐needle aspiration cytology. They provide a developmental framework with thoughtful, clinically oriented ideas for practicing cytopathologists, including a proposed list of safety‐oriented recommendations for the evaluation, diagnosis, management, and reporting of thyroid fine‐needle aspiration specimens by the cytopathology team.
