An unusual case of cyanosis and patent foramen ovale/atrial septal defect with right‐sided septal hypertrophy is presented. Mild right ventricular dysfunction led to predominant right‐to‐left shunting. The atrial defect was surgically closed. The patient is from a family with hypertrophic cardio‐myopathy;she is negative for the gene abnormality.
