Shoshin beriberi in an AIDS patient with end‐stage renal disease

Abstract Asymptomatic cardiomegaly and subsequent fulminant pulmonary edema developed in an AIDS patient with end‐stage renal disease who had no prior history of cardiac disease. Intravenous thiamine administration led to dramatic resolution of both obtundation and cardiovascular collapse within 4 hours. Prolonged thiamine administration led to decreased cardiomegaly. These findings are consistent with Shoshin beriberi.
