A patient treated as acute epiglottitis demonstrated multiple mucosal erosions on the lip, soft palate, and uvula, and constriction of his upper airway, which led to a diagnosis of mucosal pemphigus vulgaris with epiglottitis. This report aims to highlight the importance of early diagnosis and treatment of primary laryngeal pemphigus vulgaris.
