This report highlights a case of acute‐type adult T‐cell leukemia/lymphoma, which presented distinct clinical skin manifestation with erythema gyratum repens (EGR)‐like erythema. The pathogenesis of EGR remains unclear, but an obligate paraneoplastic reaction associated with internal malignancy is considered in most cases. In our case, the possibility that HTLV‐1‐positive atypical lymphocytes themselves might develop EGR‐like erythema was suspected.
