Key Points The optimal management approach of left main (LM) and equivalent coronary artery disease (CAD) has been consistently debated over the years. There are limited data on clinical and anatomic conditions influencing outcomes after elective PCI of LM and multivessel CAD. The complexity of CAD with the intention of achieving complete revascularization, frailty metrics, as well as patient's informed preference should be adequately considered in order to improve outcome even in high‐risk percutaneous procedures.
