Key Points Catheter‐directed thrombolysis (CDT) for high‐risk PE has a relatively safe profile in regards to bleeding and vascular complications Meta‐analysis of non‐randomized reports of CDT use reveals low rate of intracranial hemorrhage (0.35%), which is lower than for Systemic Lysis Further prospective randomized trials or larger scale registries—such as those proposed by the PERT Consortium—will provide a more complete evidence base regarding optimal treatment for individual patients with pulmonary embolism
