Key Points Patients who undergo non‐cardiac surgery after percutaneous coronary interventions (PCI) carry a risk of major adverse cardiac events. Certain angiographic characteristics of the treated lesions increase the risk of non‐cardiac surgery at follow‐up: ostial, distal, and/or calcified lesions. The adjunctive use of intravascular imaging may guarantee an optimal result in these lesions and reduce their long‐term risk. Left main or left anterior descending locations as well as the treatment of chronic total occlusion did not influence non‐cardiac surgical risk.
