Key Points Prehospital 12‐lead electrocardiogram should be considered as a part of the initial assessment in patients with symptoms concerning for acute coronary syndrome without causing undue delays in care. A significant amount of evidence confirms the prehospital electrocardiogram as an active contributor to the patients’ clinical outcomes. Systematic protocols including prehospital electrocardiograms should be designed within certain healthcare networks to facilitate the timely transport of the patients to primary‐PCI capable facilities. Regulatory and technological challenges exist that require a change to fully implement routine 12‐lead electrocardiograms at first contact with patients before the full potential of this technology can be demonstrated.
