Key Points This study demonstrates improved clinical outcomes with reduced D2B times in an “all‐comers no exclusion” population. Implementation of systematic, hospital wide protocols to reduce D2B times consistently across a heterogeneous population may improve clinical outcomes and reduce costs. These results should encourage registry reporting “without exclusions” and inclusion of challenging subgroups such as in‐hospital STEMI, transfer patients, and cardiac arrest.
