Key Points Advances in sheathless‐guide technology can permit left ventricular endomyocardial biopsy through the radial artery. In experienced hands, the procedure can be done effectively with concurrent anticoagulation to both prevent system embolization from the biopsy site and to reduce the risk of radial artery occlusion. The long‐term effect of repeated or single myocardial biopsy procedures using the radial artery is unknown.
